What are some of your favorite "live" mistakes during a televised wrestling show?
Of course there are the numerous mistakes in interviews (looking at you Sid.) but I also enjoy the "Oh crap, am I on?" look.
I remember one Raw where Chaz, or Headbanger Mosh, got on air, after suffering a "brutal" beating, drinking a cup of coffee. When he saw the camera, he dropped to the ground like he had been shot.
Then there was the Nitro where the sketch ended and everyone joked around until an abrupt cut to commercial. Then again, this was during the Russo era, and I can't be sure about anything from there.
Anyone else have one?

Dub-Cee-Dub running two pre-taped backstage segments out of order.

Dub-Cee-Dub running two pre-taped backstage segments out of order. That reminds me of the time Raw had a pre-taped segment with Kane attacking Mr. McMahon. Something went wrong and it started rewinding or something.

Juventud walking backwards and falling over the ring steps on a Nitro or PPV circa 1996

Booker T calling Hulk Hogan the n-word during Spring Stampede. He gets this "aw crap, I said THAT?" look on his face.
There was a RAW where the first 10 minutes were in Spanish for some reason.
Another RAW: Two Divas are talking (I think one of them was Trish) and either Steven Richards or Christian approaches them. You can hear someone off camera say, "OK, do that again" and he starts walking back the way he came.

HHH - 'I smashed Big Show's fat fist. Watch this.'
*clip airs of something else entirely*
'Ooo .... kay, that's not the clip I wanted. But what the hell, it's live TV, I'll just wing it.'

Not really as mistake so much as Flair being the god he is. Summerslam 2004 when HHH was fighting Eugene. Flair got sent backstage and dropped the F bomb 7 times.

I like it when that blood fell, and it was suppose to fall on Kevin Nash. However, it spilled on some fans sitting in the front row. Nash and Vince Russo, who happened to be there, were shocked; and Konnan and Juventud Guerrera, who also happened to be there, burst out laughing. That was so funny! It was even funny reading about it in the Death of WCW!

I remember once The Rock was doing a promo, Patterson stuck his head outside a door, had a horrified look on his face and quickly closed the door.

Well, there was that TNA Ultimate X match where the X kept falling down by itself and landed in Petey Williams hands!

What about that Raw back in 98 where Beaver Cleavage was doing an interview, and was just like "I don’t need this" and walked off, ad they go to commercial. Then they come back and Michael Cole is standing there and explains that he "Did something that wrestlers are never supposed to do, he broke character"
As far as I can remember, that’s the first time that they acknowledged kayfabe on tv
