I remember them ruining a return of a diva (either Melina or Gail Kim) from injury. It was a surprise return but they start playing her Titantron with the camera pointed at it before her music started playing.

HEY GUYS IT'S AJ STYLES! I still love how the "full video" version of AJ's debut is just full of camera cuts to the point that it's nearly unwatchable. Also, Brock Lesnar's return in 2012 where Dunn thought it'd be great to show the crowd right as Brock was going for the F5 will always annoy me.

I recall a match last year where Korporate Kane announce what was coming up, and then as the heels made their way down the ramp you could clearly see Kane walk the wrong way then turn around and go the other direction to get backstage.

Not sure if screw-ups is the right term, but during the 2002 Royal Rumble, DDP gets tossed out by Christian... Which we don't see, because the cameras spend almost the entire 90 second interval on the Godfather and the ho's.

His name plate when he made his entrance listed him as the Champion.

Kane's pyro didn't go off at Royal Runble 1998 and it kind of undercut his attacking Undertaker.
Also they cut to Kane running down the ramp at Judgment Day 2001 and missed Austin pinning Undertaker.

There was a PPV in 2000 where Billy Gunn was challenging for the Intercontinental Championship. His name plate when he made his entrance listed him as the Champion. Because he WAS the champion. He had an injury for most of 2000 so the only PPVs he did were Royal Rumble, No Way Out, No Mercy, Survivor Series and Armageddon. He was still doing New Age Outlaws stuff at the first two, at No Mercy he tagged with Chyna against Right To Censor, at Survivor Series he was in a big elimination tag and at Armageddon he lost the IC Championship to Benoit (having won it on TV after Survivor Series from... I think Eddie Guerrero).

The TNA thread shows the commentators reading from scripts, and during the Shane/Taker match when Shane jumps from the cage you can see Cole reading from a script.

I think it was after Triple H turned on Ric Flair, and H goes to show something on the Titan Tron and the wrong clip is played. Afterwards H says something like, "Well that was the wrong clip, but hey it's Monday Night Raw so we'll just go with it."

That one was mostly on Michelle for saying "LOSER LEAVES SMACKDOWN." You'd think they would have her actually scream WWE into a mic instead of using such an obvious wildline. I think the plan was for it to originally only be loser leaves smackdown but then Michelle wanted to retire. At least that was my memory of it. And you'd think they could have just left it at "loser leaves Smackdown", then between the promo and match, have Michelle cut a quick promo or something saying "you know what, if I can't even beat you, forget Smackdown, I'm leaving WWE" or something else to that effect. It would be less embarrassing and could probably add to the characters involved.

Of all the screw-ups over the years, this is the one that immediately comes to mind for me. Truly representative of the chaotic, self-aware mess that was Season 3 of NXT. That was actually during the brief Big Show feud for NYR 2006, unless it also happened during the Flair feud.

The introduction to WrestleMania XX showcased the set, only for the animation meant for the screens on the right side of the set to appear on the left side, as well, creating an asymmetrical look.
At WrestleMania 22, during Finlay's entrance, the Willis Tower in the chyron was much smaller than usual.
At Vengeance: Night of Champions, JBL is mentioned as something along the lines of "Longest reigning World Heavyweight Champion in past decade".

Because hey, not like TNA has the market cornered on it. Besides most of the career of Lillian Garcia, I can think of a few others offhand. One that comes to mind is one of the segments during the Kane / Cena feud where they cut to the back early and you could see someone poking Kane with a script telling him to step into position. Another coming immediately to mind... That Kane(and I think Eve too) backstage one is a classic.

There was an episode of Smackdown a while back that aired with with the producers' talking to Cole etc being broadcast a couple of years back wasn't there? It was an episode of NXT season 3 that was posted to their Youtube (as it was the only place to really watch it) that had all of Kevin Dunn's cues and comments. Also, the mic died during one of Triple H's last promos before Unforgiven 2003 which led to him having to test a mic in the middle of the ring to make sure it worked and then commented that he didn't know that Goldberg made microphones.

When Shane dived off the cell Michael Cole was shreiking and holding a piece of paper and looking down at his lines. It looked realy amateur.
