When Shane dived off the cell Michael Cole was shreiking and holding a piece of paper and looking down at his lines. It looked realy amateur. He really had to make sure he got that "For the love of Mankind" line in there.

Didn't the first One Night Stand air a backstage bit of Roadkill (I think) doing the RVD taunt too early in the night, it seemed like it was supposed to be shown right after RVD's segment

I remember an episode of Raw where they cut to the Shield waiting to make their entrance, ruining the surprise and Cole covering for it by saying they were plotting destruction and a camera guy caught it or similar.

I think it was mentioned before that at the end of a Mohammed Hassan match they accidentally played Charlie Haas' theme, which was weird because he either wasn't on the roster or Raw at the time. And didn't Roman's pyro take forever to go off at 'Mania this year?

Kane/Eve with the ambulance segment starting with Eve carefully climbing into the ambulance... which looked like it cut to the back too early.

I remember one when Kane and Big Show were the tag champs, and WWE showed their entrance on WWE Unlimited. Kane did his customary motion just before his pyro goes off ... and nothing happened! Which left Kane with this "aw crap!" look on his face.
It may not be a production issue, but Michael Cole giving away the outcome of WrestleMania XV's main event has to be up there if we're talking screw ups. I would have loved to see Vince's reaction when he heard that!

I believe Cole was shilling the WWF doing something on the Home Shopping Network, mentioning that the new WWF Champion would be appearing after WrestleMania.

Somewhere at Survivor Series 01 they screwed up and both sets of Tag Champs were introduced as WWF Tag Champion. By the DVD edit, this had somehow migrated to them being treated as completely the wrong way around, and the Hardyz were introduced as WCW Tag champs and had those entrance graphics, despite it being fairly obvious they were wearing the WWF belts, and Dudleyz were holding the WCW belts. This still isn't fixed on the Network.

They also had the titantron framed wrong, so when the name JERICHO comes up, all you could see at home was ERICH. I just watched the video again. You're right. What *happened* with that segment? Was the regular production team on vacation? Being a big Jericho mark, I was pissed when that whole segment was botched.

I was ringside with 3 of my buddies getting quite tipsy while living through a live episode of NXT Season 3. I can't do justice to how funny we found this and it's still referenced regularly to this day
