Because hey, not like TNA has the market cornered on it. Besides most of the career of Lillian Garcia, I can think of a few others offhand. One that comes to mind is one of the segments during the Kane / Cena feud where they cut to the back early and you could see someone poking Kane with a script telling him to step into position. Another coming immediately to mind...

Remember that guy who tried to burn Undertaker alive?
Not Kane.
The guy at Elimination Chamber.
... was it Kane?

Obviously, these are considerably less often and embarrassing than TNA ones, but Undertaker's had some awful examples with his pyro, between when they had Kane's go off in his face by mistake and when he was set on fire with his. Triple H getting burns from dry ice also would count.
Not an error, exactly, but whoever it is that thinks it looks great when a camera goes up and down with a wrestler's fist when they are mounting someone and punching needs a boxing glove to fly out of their monitor and bop them in the face, because holy hell, is it awful.

There was that time when the one camera became the Worst Case Scenario...

Ugh, yeah. In general in recent years WWE's camera work is atrocious. Though still better than TNA's. Oh, another random one I just remembered, when they suddenly started playing someone or other's music - wanting to say Big Show - mid-match before the guy had actually won.

The entire first Jericho return was shot awfully.
They blatantly showed him on camera when he attacked the runner (where you weren't supposed to see his face), then INTENTIONALLY showed him doing his pose, ruining the surprise. Finally, the camera missed his turn towards the camera.

Credit to Taker for hardly flinching when that happened. One odd thing I did notice when watching it back (July 13th '98, #268 on the network if anybody was wondering) was that it was only the one in the corner where he was standing that was set off, Kane came out just after and did the other 3.

I just watched the video again. You're right. What *happened* with that segment? Was the regular production team on vacation?

There was an episode of Raw in 2003 that began with everybody speaking Spanish.

Am I remembering wrong, or did Jericho with Fozzy once perform on Sunday Night Heat where there was ZERO audio? I wasn't sure whose fault that was, if that happened, might have been on MTV's end, to be fair.

I think it happened in the New Age Outlaws v Mankind/Kane match at SummerSlam 1998. Also a Rob Van Dam match, but I have no idea on the timeframe. EDIT: Definitely on the Outlaws match. Happens after they put Mankind through the two set up chairs and Road Dogg goes for the lazy pin. Mankind kicks out and you hear the opening riff of the Outlaws theme.

Royal Rumble 2011
Hornswoggle goes up to hit the frog splash on Slater while Cena stands in the corner watching. Hornswoggle jumps, the camera switches to Cena and switches back after the move is done. Cole confirms the move was successful.

Rumble are prone to this kind of thing. Just this year they missed Jericho eliminating Kingston off of Big E's shoulders because they were too busy focusing on Reigns getting beat up by The League Of Nations.

I don't know if this one counts as a production screw-up but I am sure they could have announced something before the PPV to avoid this piece of amateur hour... www.youtube.com/watch?v=-dzJ9aNfmbM

You'd think they would have her actually scream WWE into a mic instead of using such an obvious wildline.

I think the plan was for it to originally only be loser leaves smackdown but then Michelle wanted to retire. At least that was my memory of it.
