I figured the fall would have been fatal anyway unless Trevelyan got immediate medical attention, but the cradle falling on top of him was technically the fatal blow. At least in the game, it's the fall that does him in.

At least in the game, it's the fall that does him in. True, but this is Bond, which generally goes by the "if you don't see a body they're generally alive" trope.

Not just Batman 89. In Batman Forever, Two Face falls into the wreckage of that giant metal structure. In Batman Begins, Ras Al Ghul is in a train that crashes off of a tall rail. I'm probably stretching it, but a type of fall does happen.

Also, one movie he was in was the second sequel, and the other movie he was in was the second movie. Batman '89 doesn't count because he wasn't Two-Face yet.
