That area that you end up going at the end of Arkham Asylum. I'd go in there multiple times, and nothing was there but a dummy dressed as Joker with a tv on its head with a recording of Joker laughing repeatedly. Every time you entered and exited, the Mannaquinns would be different...sometimes one, two, or three, ect. Just creepy as hell. Never been psyked out in a game like that before.

I can't find a YouTube clip of it, but in Resident Evil 4 pretty early in the game if I remember right. You are in a little cabin, and when Leon turns around to exit (I think) the camera shows a body being held up on the wall by......a pitchfork I think. That part caught me off guard like 3 times before I remembered to remember it was there.

I don't know if "scary" is so much the right word, but the scenes with the fake Col. Campbell inside of the Arsenal Gear are just unnerving. If you don't see it coming, which who would, it really catches you off guard and creeps you out.
Also from Metal Gear, when the four attackers in the stealth suits are in the lift with you, and Otacon finally realizes it.

All of Silent Hill 2, most of the scenes in Condemned: Criminal Origins, all of Dead Space. Dead Space was so damn creepy I took a long as time beating the game as I would walk VERY slowly.

A game called Iron Helix can have some creepy moments; it's kind of like a cat and mouse game. You have to travel through the ship while avoiding a robot thing that is set out to destroy you.

I nominate any time you go through the woods or the deserts in RDR, it's all silent and suddenly a Cougar sprints out, kills your horse & then you before you can even get up.

Of course there was the first time you hear the breathing and footsteps on Doom. That freaked most people out.

Resident Evil 4
When you get in the castle and Ashley first gets caught in a trap. You have to go down into the sewers and fight those weird insects. When you run down the first screen, you can hear the sound of someone running up behind you. You turn round and there's no one there. Works even better in surround sound.
Eternal Darkness
When Alex goes to investigate a bathtub in the house and the camera really quickly zooms into the bathtub and it contains a dead Alex sitting in a bath full of blood.
Shadowman
The "Playrooms" Level. Where the music has the sound of squeaky toys and children laughing and screaming. Some rooms in the level have children's playpens and letter blocks.

Diablo- Was playing it with a friend long ago and we were about to fight the Butcher. And we were walking around looking for him and there was no sign of him anyw-FRESH MEAT! AHHHHH!!

Just about all of Eternal Darkness.
Dead Rising- The first time I opened the warehouse elevator, and the zombies poured out, I nearly crapped my pants.
Resident Evil 4 was creepy in general.

Here`s one most of you probably never played. Clock Tower. It is considered a crappy game, but when Scissor Man comes after you, it is intense.

Oh, and the entire game of Dead Space is a scary moment! They made Twinkle Twinkle Little Star a scary sounding song!

In AvP2 when you're playing through as humans and the first mission or two there are zero enemies. I mean that literally, there are no enemies. It really builds up the tension for when you're finally under attack. Oh god, that game shellshocked the hell out of me. It wasn't scary when the aliens actually showed up. It was scary when the motion detector was going off and then nothing was there. Easily the best FPS of 2009 but people look at me like my head is on fire when I say that. I don't know about that, Clock Tower is a pretty popular series.
I had a few moments in FEAR and FEAR 2, Condemned 2. Pretty much any Monolith game.

It is scary and the enemy AI is great. Everything else needs work. I really hope the third one is developed better.

On the note of Dead Space ( Which is arguably the best TPS ever in my opinion) How about any of the death scenes? Those are scary as hell and ypu can see in the necromorphs face, a look of almost hunger..

20 minutes into a battle on Fire Emblem. My front line is breached and they can reach the freaking white mage. God, that sort of thing is always a nailbiter for me.

The scene in Silent Hill: Homecoming where you have to decide whether or not to shoot the main character's mother in the head to spare her the agony of being ripped in half.

The one that stands out is the first time you go into the Other World in Silent Hill, and you're in the school courtyard. All of a sudden, music starts off that is literally just two quick bangs followed by what sounds like an animal growl. I got so scared that I ran for a door, but the first one I tried....
WAS LOCKED! NO! PLEASE DON'T MAKE ME CROSS THE COURTYARD! THERE'S A MONSTER OUT THERE!
It was all soundtrack, but it was just brilliant.
Also, playing RE4 on Professional Mode when you're low on ammo is always scary, especially in the part with the bugs that camouflage. Now, THAT is survival horror.
