Lord Frieza from Dragonball Z for similar reasons. Yeah, I was thinking Frieza, too. I wouldn't have guessed at the time that it was possible to make me to feel bad for Vegeta. Frieza was so freaking evil that he did the trick.

Adrian Veidt mostly because his actions are almost justifiable. Which adds another element to just how evil he really is. His actions weren't random or chaotic, but meticulously planned. I wouldn't call Veidt evil considering all his actions(as horrible as they were) were done for the greater good of mankind, it's not like Comedian who was purposefully an amoral bastard.

Patrick Bateman from "American Psycho," the book in particular. I honestly can't think of anyone that comes even close.
A serial killer who rapes, tortures, dismembers, and murders his victims, and then sometimes eats and rapes their corpses.
He kills men, women, and animals with no remorse, women because he gets a sense of sadistic pleasure out of it, and men because they annoy him.

I think you could make a case that Governor Tarkin is the most evil character in Star Wars. He's not even the main villain in charge, yet he decides to destroy a planet and kill billions of people just to make a point, and shows neither reluctance nor remorse. Even Palpatine never did anything THAT f***ed up.

There's a sequence of deleted scenes from Return of the Jedi on the Blu-Ray set in which The Emperor orders the Death Star commander to turn the Death Star around and destroy Endor if the Rebels take the shield generator out that are absolutely awesome. The Death Star commander is seriously conflicted on this and it really shows what a bastard Palpatine is..........I actually wouldn't mind seeing that re-inserted into the film next time George decides "No, really....THIS is how I always envisioned them!"

How about Angelus from Buffy/Angel? Referred to in the series as the most evil vampire in history. I still to this day think he had the greatest heel turn in a tv show.

EDIT: Damn, beaten to it by a single minute! And even though it turned out that Scott's father was actually his own father too, he wasn't even bothered by the fact that he had him killed. He was more bothered that he was ginger.

Professor Moriarty was pretty evil in Sherlock Holmes: A Game of Shadows.
I am not sure how familiar anyone is with the graphic novel "The Preacher". But Jesse Custer's sadistic grandmother and Jody were the epitome of absolute evil too.
