He was a pretty good wrestler, had a cool gimmick (being in the mafia), had an awesome theme song, and spoke with that stereotypical Italian accent. I was a fan. What went wrong?

He was basically just a notch above being TL Hopper/Freddie Joe Floyd/Alex 'The Pug' Porteau/The Goon, so it doesn't seem they had any great designs to get him over. I think Brandi had a good look but he comes across as kind of a dick in most interviews I've seen.

It's a shame too, because say what you will about the guy, but Brandi was talented in his day. He wouldn't have ever been a Hogan or even a Savage/Warrior, but I could easily see him being at the very least a solid mid-card talent, at best, likely IC title tier talent.

He could have absolutely worked had he arrived two years earlier (when he would have been at the top of the more cartoon-y performers) or two years later (when they could have tweaked it as a debuting Attitude character), but the summer of '96 just has too many disposable debuts (Freddie Joe Floyd, TL Hopper, the Bruise Brothers, Alex Porteau, The Goon) to make Sincere's two-month-long undefeated streak stand out.
His promos that summer about how much Bret Hart reminded him of himself were great. Unfortunate that after his streak came to an end at the hands of the Undertaker (a match hyped to make him seem like a credible threat in the main event of Raw) they did absolutely nothing with him until the Mero "shoot" angle.

I remember seeing him at the 1996 Survivor Series he sucked.
The next time I saw him, he was Tom Brandi in the 1998 Royal Rumble and, despite looking great, he still sucked.

I still to this day have no recollection of Tom Brandi ever existing back when I watched in 1998. I can at least kind of remember every other person in the 1998 Royal Rumble except him.

I always took it that he was a mafioso. His theme music was something out of The Godfather. His thick Italian accent, and the way he would say, "Sincerely!", gave off the impression that he was trying to deviously gain other people's trust, like a shady mob guy would.

Even as a kid I knew all the guys debuting at this time completely sucked. Alex "The Pug" Porteau, The Stalker, Freddie Joe Floyd, Who, The Goon, Leif Cassidy, T.L. Hopper, Avatar, Mantaur, Faarooq Asad, Tekno Team 2000, Zip, Fatu "man of the streets", The Head Bangers and, the man in question, Salvatore Sincere. He came up at a bad time where guys were being saddled with awful gimmicks. Some, like JBL and Austin and Rock were able to recover, but most were not so lucky.
And for the record I never even considered that Sincere was supposed to be a mafia guy until right now. I'm not so sure I'm sold on that one.

to this day I have no idea what his gimmick was even supposed to be. if he was supposed to be a mafioso he sure didn't look like one. I can't remember too many guys in the Godfather dressed up like the world's gayest rodeo clown.

Have no idea where you got the mafia idea, not all Italian gimmicks involve the mafia. He did have that Rick Martel vibe.
Saw his shoot from 07' I wanna say and he still was in really good shape. Seemed like a good guy, remember some disagreed with his view on the Royal Rumble as he preferred being eliminated right away instead of lasting longer as every guy got the same payoff.

That's a very good question: What WAS his gimmick supposed to be? I remember when I saw him I thought it was just a hack version of Rick Martel with a bad accent.
