Title.
I remember it happening in TNA with Kurt Angle and Hulk Hogan and Rock/Foley in 1998, but I'm certain this isn't the only time this has happened.
And a side question, how many times has Earl Hebner repeated this or used it to get himself over?

Screwjobs in the title match/main event were actually a running theme for quite a long time at Survivor Series.
In 1999, Vince screwed over HHH (ironically Vince was a babyface at the time).
In 2000, Angle screwed Taker with the Eric Angle plan.
In 2001, Angle was the WWF's double agent in the Alliance.
In 2002, Heyman screwed Brock and defected to the Big Show.
In 2003, Kane helped Vince win Buried Alive against the Undertaker.

RAW April 1998 - Dude Love vs. Steve Blackman (at Unforgiven, Vince told Mark Yeaton to ring the bell when Dude had Austin in an abdominal stretch but Yeaton didn't notice) Survivor Series 1998 - Rock vs. Mankind Starrcade 1999- Hart vs. Goldberg RAW May 2001 - Austin vs. Benoit Slammiversary 2006 - This one didn't involve a sharpshooter, but had Earl Hebner purposely cost Christian the KOTM match and allow Jarrett to win. Breaking Point 2009 - Punk vs. Undertaker TNA Impact 2010 - Styles vs. Angle

Punk/Taker has to be the worst one. It didn't make any frickin' sense for Long to turn on Taker.

Punk/Taker has to be the worst one. It didn't make any frickin' sense for Long to turn on Taker. It seemed to be building towards an Undertaker vs. Vince storyline, presumably with a match at Mania, but then Bret decided to come back and so they just forgot about the whole thing.

Punk/Taker has to be the worst one. It didn't make any frickin' sense for Long to turn on Taker. It seemed to be building towards an Undertaker vs. Vince storyline, presumably with a match at Mania, but then Bret decided to come back and so they just forgot about the whole thing. They were insinutating that Vince forced Teddy to screw Taker, but as was mentioned, they struck a deal with Bret which is where any real money in a Screwjob storyline would have been with.

Punk/Taker has to be the worst one. It didn't make any frickin' sense for Long to turn on Taker. As bad as that was (and it was really bad) I think the biggest anger I've heard toward any usage of the rehash was the Styles one from Impact in 2010 because it was a malicious screw-job apparently done entirely in a jovial babyface manner, making who to root for flat out confusing. Can anyone verify this?

I think that WCW did it with Sting and Goldberg at some PPV but I'm not sure if I'm remembering that correctly. I think that it was trying to set up a Sting heel turn.

I went to a TNA show in Brussels, Belgium in January 2011 and when Hebner showed to referee his first match of the evening, he wore Bret sunglasses and a shirt that had 'I screwed Bret' written on it (as in actually written with a marker). The show was very poorly attended, most people in the crowd had no idea what this old referee was doing and the ones who did didn't care. It came off as desperate, pathetic and completely out of touch.

Also, when Shawn Michaels was facing Shane McMahon on an episode of Saturday Night's Main Event, Vince McMahon rang the bell when Shane put HBK in the sharpshooter(I think thats what happened, not 100% sure).
