Subverting a little as its a non death but after reading about the Red Wedding from Catelyn's POV where as stated above she watches (who she believes to be) her last son die before getting her throat slit we jump straight to an Arya POV chapter where she gets knocked out by the Hound trying to run towards where Robb is.
Before he went all lazy bones GRRM had a tricky way with words so (going from memory here, don't have the book to hand) her POV chapter ends with something like "as she ran she felt the ax hit the back of her head and everything went black".
Turns out that he hit her with the flat of the ax and she lives to see another day but it was written to give the reader the sense Arya was dead, killed just as the reader is trying to process the deaths of Robb and Cat. Threw the book across the room and went outside for a smoke (have long since quit) I was so pissed off.

Robb and Grey Wind dying in A Storm of Swords was heartbreaking but, the final blow for me was Catelyn. Having just gotten to this point, I had the opposite reaction. Robb came across as stubborn and stupid throughout the story, and Catelyn came across somewhat cold. There was something quite unlikeable about both for me. Then again, I find Tyrion, Jaime, and Arya far more interesting and likeable than Rob, Sansa, Bran, Jon, and Dany, so maybe I'm just not invested enough outside of those three.

Ellen Brody exploding Jaws The Revenge with a boat. {Spoiler}some TV versions of it has the shark not exploding and just sinking to the bottom of the sea after losing so much blood, kind of like the original ending to JAWS where the shark is killed by multiple harpoon inflicted wounds and sinks to the bottom of the ocean but dragging Quint down with it as his foot is caught in a rope tied to the harpoon stuck in JAWS Also for me it was the way they had Cam and JT die in Degrassi. I mean with Cam it pissed me off that they focused on him being homesick and just not liking that his teammates dick around with him isn't in my mind a reason to commit suicide. Now I am in no way knocking suicides because I lost a lot of friends to that due to depression but I just feel like there could have been more to it. Like show more to his depression to make it more impactful. JT's death on the other hand pissed me off more because I wanted that soulless bastard that killed him to get more than just a passing reference with saying he is in jail.

in the second ghost recon book, combat ops joe ramirez who was scott mitchell's kind of right hand man's death pissed me off cause he was cool and i liked him from the first book, plus the way he went out was just dumb

Jimmy Olsen in Batman V Superman. He dies with barely even appearing simply because snyder was "it's a swerve ,bro!"
