So we’re also discussing replacing different characters with a similar role instead of recasting the exact same character with another actor? In that case, bringing in Matt Berry as the Boss in The IT Crowd is my pick. Chris Morris is a genius and I really liked him as Denholm but Douglas is so much funnier. He got a hell of an exit too

There's an unaired GOT pilot episode where there's a few cast changes from what we ended up with, most notably with Tamzin Merchant playing Daenerys.

When Marvel first got the movie rights for Iron Man back from, I think, Universal, Marvel had absolutely no background in making their own movies and weren't really attracting any A-level names to the project. Finally they came across Terrence Howard who was interested, and they gave him a big money deal to play Rhodey, probably more then he was actually worth, with the hopes that Howard's name attached to the project would bring in other top names. That didn't really work out. Eventually Jon Favreau and Robert Downey Jr. agreed to do the movie, completely independent of Howard, simply because they were fans of the character. They wound up getting paid much less than Howard, and even when other big names like Bridges and Paltrow jumped on, Howard was still easily the highest paid actor in the first movie. The movie was a success and Marvel made plans for a sequel, but insisted on cutting Terrance Howard's pay. Now to Howard, he had just starred in a wildly successful movie that just made a gajillion dollars, he was being asked to return for a sequel with his part being significantly expanded, and was told he needed to take a pay cut to do it. To him, and probably most actors with any name value, this would sound ridiculous, so he walked, and the part was recast with Don Cheadle. Also remember at the time Marvel Films was under the direct control of the notoriously stingy Marvel CEO Ike Perlmutter, so that probably added to the issue. If I remember correct they very nearly lost Samuel L. Jackson for similar reasons. I believe that Don Cheadle was also their first choice for the role to begin with but was not available, so there is that as well.

Star Trek. Kirk wasn't the first captain. Pike was just in the pilot. ( well originally) Cast changing from pilot to series is pretty common. One of my favorites was According to Jim. For the pilot they had Sam Seder (yes, the political commentator) playing Andy. This was during a long stretch where TV producers were absolutely convinced that they could make him a sitcom star, putting him in pilot after pilot. They replaced him with Lawrence Joseph Campbell for the actual show since, you know, the role kind of required you to look like he could possibly be the sibling of Courtney Thorne-Smith and Kimberly Williams-Paisley.

Star Trek. Kirk wasn't the first captain. Interesting enough, in Star Trek lore, Kirk was the third captain of the original Enterprise. The first was a guy named Robert April (and he was the name of the captain on the script of the original pitch for Star Trek), while the second was Christopher Pike as played by Jeffrey Hunter. They brought back Pike and Hunter for "The Menagerie," which was half-new episode, half-repurposed cut of the original pilot, "The Cage." It was also the first two-parter in franchise history. April was mentioned in Discovery and also appeared as a commodore in The Animated Series, where he was voiced by James Doohan. Then again, wasn't practically every character in the Animated Series other than the main cast was voiced by Doohan?

By the same token. Zachary Levi (who was originally supposed to be Fandral in Thor 1 but couldn't due to conflicts with filming Chuck) played Fandral in Thor 2 and 3 replacing Josh Dallas (who had to leave due to conflicts with filming Once Upon a Time) >_> "Playing Fandral in Thor 3" aka being the sheathe of Hela's dagger and not getting a single speaking line, sure. (Still bitter about how dirty Sif and Warriors Three were treated in the entire franchise out of the first movie) Can't argue against that. At least Sif got to show up on Agents Of SHIELD, and Hogun got to die a proper warrior's death trying to defeat Hela and her armies, but I still miss those four. I think they were my favorite ancillary characters in the entire MCU. If we're handing out series on Disney+, let's get them a miniseries or something!

I thinkt Waititi/the Russos made a reference to Sif not being among the dead Asgardians. Possibly because Jaimie Alexander wasn't available due to scheduling conflicts and they didn't want to kill her off screen.

Halfway through the third season of Monk, they lost Bitty Schram (Sharona) and replaced her with Traylor Howard (Natalie). The show went on for five more seasons.

The original plan for the animated series even had Doohan doing the voice for Sulu and Majel Barrett voicing Uhura. Nimoy refused to participate unless Takei and Nicholls were brought back.
