An obvious one but what if Kurt Cobain had lived? the opposite end of this sort of question, what if Bob Dylan died in his motorcycle crash in 1966 a week after releasing Blonde On Blonde?? What if Aerosmith buys the plane that ultimately crashed with Lynyrd Skynyrd onboard?

What if Marvel had given a big push to their creator owned imprint and improved their royalty system in the 90s to the point breaking away was no longer an easy decision for the Image founders?
No massive #1 issue fuelled comics boom and subsequent bust, Valiant wouldn't have been so badly wounded by the Image crossover, no firesale of movie rights from a struggling Marvel. No toybiz merger so no Perlmutter screwing with the books and movies. No Marvel legends, no MacFarlane toys either so collector toys would seem much different. No four horsemen sculpting Matty toys like MOTU either as they came from Todd's company and so on.

Vince going to prison and Jerry Jarrett running the WWF and presumably no attitude era, no MNW, a downsized/bankrupt WWF and potentially a more relevant ECW. How many lives/fates does that change?
In 1999 a cash strapped Marvel offered the rights to a load of characters (Iron Man, Thor, Black Panther ect) to get as much as they could from the Spiderman sale but Sony passed as these characters hadn't really broken beyond their core audience of comic book fans and were virtually unknown outside of the US.
The dead at 27 club, who would have made the most impact on the world had they survived? Who would have done a Morrissey/Clapton and grown old disgracefully?

An obvious one but what if Kurt Cobain had lived? How would Nirvana be regarded today? What would their post-1994 records sound like? How would Dave Grohl's career have played out? I've always had a feeling that if Kurt lived maybe Nirvana take a sabbatical for a few years and the Foo Fighters become Dave's main focus anyway I've always understood it that Dave was starting to have a foot out the door around the time Kurt died.

What if the Jim Henson Company bought Disney in the early '80s? Other way around. Henson wanted to sell The Muppets to Disney. He wanted to move on from them to do more passion projects. Hell, he only agreed to do Great Muppet Caper amd Muppets Take Manhattan to raise funds for his dream projects: Dark Crystal and Labyrinth. So, the question is "What if Jim Henson had lived?" What films would he have done with the money from the sale of The Muppets? Only one of Henson's post-Muppets projects ever saw the light of day: A TV show about a family of talking Dinosaurs.

What if Marty McFly didn't go back to the old west for Doc? 1955 Doc could just not invent the time machine and therefore would never had ended up in 1885. That could have been one of those universe ending paradoxes that Doc was always warning about. It was only Copernicus finding the grave that convinced Marty to go back to 1885. If they had left the dog at home then that would have been that.
