Are we talking about different names, different personalities, or both?
Sonic characters technically fall into both categories, with the Eggman/Robotnik split between East and West, and personalities of many of the main characters often differing between adaptations or canons.
The infamous dickery of the title character in the Sonic Fleetway comics might be one of the most notable.

If we're just talking names there's the Sinister Six, which, for some reason, the 90's Spider-Man cartoon called the "Insidious Six".
And then on JLU the Suicide Squad would never actually be called that, instead opting for their official name of "Task Force X".
If we're talking personality, then Raphael of the Teenage Mutant Ninja Turtles is the king of this. While he's the "rebel" of the group, practically every different interpretation of him depicts this differently. Everything from a snarky slacker in the 87 cartoon to an almost uncontrollable psycho in the original Mirage comics.

Hellfire Club is generally called something else because the dont want to use the word hell.
Same thing with Suicide Squad. Using that word is generally frowned upon in cartoons.
The Sinister/Insidious thing was just Fox being stupid. Like no punching or landing on pigeons.

The West series never really gave most of its villains real names/secret identities, so they never said the Newmar/Meriwether one was Selina specifically, nor who the Kitt one was. I just decided that Kitt was a copycat, no pun intended, crook. Like the John Astin version of Riddler. (If anyone, Gorshin = Edward Nigma). Now if you were thinking of hair color, it's because Fox or WB insisted she be blonde to match Michelle Pfiffer. Anyway fun topic, I'm going to come back when I think of some.

Well, there is the Harley Quinn series Catwoman, who IIRC is Selina. And speaking of...there's Harley herself, but her personality depends on the medium she's in. That and whether her skin was acid bleached like Joker's or she just paints herself up.

Speaking of Joker...former Red Hood who took an acid bath, disgraced stage hypnotist, ex mobster Jack Napier, anonymous anarchist in face paint, Arthur Fleck the embittered comedian, etc. etc.
Penguin = British mobster? Mutant penguin humanoid? Italian mobster?
Batman himself fits this trope. Most live action movies downplay his detective skills and make him a pure action hero. Let's not get started on the kill or no kill thing.
Rocket Raccoon in MvC3 and EMH having the British accent vs. the Brooklyn one he is now known for.
Cobra Commander: embittered ex car dealer? Humanoid snakeman from Cobra-La?
I see this a lot, actually, when you go from Disney theatrical to TV, having been watching The Disney Afternoon shows:
Chip and Dale: antagonizing prankster antiheroes? Heroic detectives? TV stars who had to get back together to save their costars?
The Jungle Book stars who crossed over to TaleSpin. Where Khan goes from villain to ruthless businessman, for instance. And Baloo and Louie being actual friends...was that more a TS thing?
Popeye is sort of like this. Comic Popeye was not a wimp until he ate spinach. Not all his stories were competing with Bluto for Olive Oyl. Heck, Bluto was a one shot villain, and Brutus in the comics is more dumb muscle than malicious.
Can we count Raccoons specials vs. series? Because....Cyril Sneer. Just Cyril Sneer. Cedric becomes less of a wimp on the series. And Melissa becomes an independent career woman, not the quiet wife of the specials.

Just for whatever reason, TAS pronounced it "raysh." That's a mispronunciation. It's a possible case of Orientalism because it's an Arabic title, so maybe changing the pronunciation from "ras" to "raysh" was to make it sound more mysterious or whatever.
The original Jungle Book and how it differs from Disney's is an interesting subject. Like in the Kipling book, Kaa is not only not villainous like the Disney version, but is one of Mowgli's mentors and friends. He's also friends with Baloo and Bagheera.

The TV version had DAVID Banner become the Hulk when he experimented on himself about the human psyche. Comic Hulk is more rambunctious and simple minded, with his famous Hulk Speak. TV Hulk is more like Frankenstein's Monster and never talks. MCU Hulk is kind of a combo of both versions.

Dumb Hulk who wants to be left alone is the most famous depiction but at other times he has been incredibly intelligent. There was even a period where his transformation wasn't based on anger. Modern cartoons and movies tend to play up dumb hulk, likely for balance. If you have someone as smart as Banner in a body that can beat up thor, would he really need the others?

One that comes to mind and one that greatly annoys me involves the Silent Hill movie.
In the first Silent Hill game the protagonist searching for his daughter is Harry Mason, but in the movie the protagonist searching for her daughter is Rose Da Silva.
I don't care at all about the gender swap and I actually really enjoy Radha Mitchell's performance. What gets me is that for some inexplicable reason they change the name of the missing daughter from Cheryl to Sharon.
I understand the reasoning for changing the gender of the main character and can get behind that, but why make such an innocuous, non-important change like that? It doesn't impact the telling of story in any way, it doesn't mean anything to people that haven't played the games and all it does is stand out like a sore thumb for anyone that's familiar with the games.
