Two episodes of "M*A*S*H" come to mind... the one where the ghost of a dead soldier wanders around the camp trying to communicate with everyone (and only Klinger can but he's delirious with fever), and the one where the cast all have dreams that become nightmares.

Steven Universe going for a checkup at a doctor as he had never had one before, and discovering his skeleton has been shattered numerous times over the course of the series, including his skull, and that he'd only survived due to his powers. His bones are shown onscreen melded back together with crystals, reminding the audience a teenager has spent the entire series being beaten to death, falling from great heights, or otherwise suffering trauma he's forced to power through and continue.

the one episode character Sylvia
teen, changing in her own bedroom and spied on by teenage boys, accused of luring them there, no adult condones the boys
she is walking home and gets sexually assaulted by someone in a clown mask, she tells her Father this but tells her not to tell anybody because it will risk HIS reputation
She gets pregnant because of said attack, someone overhears it and gossip spreads. Turns out Father blames her for the attack (you reap what you sow)
she runs off and is again attacked by the masked assailant, but manages to fight him off in a barn
and oh yeah... at the end of the episode she dies after dreaming of the perfect life and wedding

Both messed up episodes of Punky Brewster. Describing that one of the girls got locked in an old refrigerator and almost dies would make someone think you're just making stuff up. Same with the cave episode where she fights evil spirits.

Most Captain Planet episodes, due to the nature of the villains they fight, but the ones that turn out to be dreams, especially. Wheeler being captured by tiny rat people; Wheeler and Linka being married with a bunch of kids and all the Planeteers, Gaia and Captain Planet are dicks now; and the one where they go on a game show playing against the Eco-Villains.
Also, the episode where Linka does drugs and her cousin dies, which, canonically, ACTUALLY HAPPENED.

Off topic, but there's a series of Garfield strips, written by Jim Davis himself, that are about Garfield waking up in his house alone and experiencing some serious trauma upon realizing that. No jokes, just the titular character having a complete breakdown.
Ends on the weird note of implying the mainline strip is just Garfield's dying wish of what he wanted his life to be.
Really weird stuff that as far as I know the author has never been willing to discuss.

The Changing Nature finale of Dinosaurs sounds exactly the sort of dark/depressing finale a fan would invent for a relatively light hearted Jim Henson sitcom... except it actually happened.
