I heard that several episodes from the Outer Limits from season 7 were meant to have spin-offs, I think the one with the time traveler, the alien shop owner and the the trial in space were supposed to be spin-offs.

There was that backdoor pilot for the Supernatural spinoff set in Chicago. There was also Buffy the Animated Series, Ripper, and Faith the Vampire Slayer all from Buffy of course. That was the worst supernatural episode ever. Why given that it was a back door pilot did they only hire people who could not act? Buffy the animated series looked great i really wish they had made it. Gilmore Girls had a backdoor pilot for a show featuring Jess with a wacky family in California. It would have been absolutely horrendous. I did not watch Supernatural until its second season because I thought it was a spin off of Gilmore Girls.

There was a live action version of Monsignor Martinez filmed back in 2000, but it didn't get picked up and never leaked online.

I always heard they wanted to do one set in the 60's with Blackadder as the manager of a popular 60's Beatles-esque band. They gave up on it when Rowan Atkinson decided he was too old to do the part anymore. I'd say do one set in the 80's where they are all part of Margaret Thatcher's cabinet. You can blame Alan B'stard for that not happening.

The Joey spin-off from Friends. Obviously, they decided at the last minute that even attempting such a thing would cheapen the legacy of the original and it was a foolish idea from the beginning. Therefore, it NEVER. HAPPENED. I liked Joey. You liked what?

The made for tv movie: Return of the Incredible Hulk was supposed to also be a pilot for a Thor series that never saw the light of day. A shame too, because while I'll always prefer the comics classic version of the God of Thunder, the Hulk movie version was very fun and interesting, as was his human counterpart, Don Blake.

For a more recent one, there was supposed to be a phineas and ferb spinoff involving the fireside girls, but with P&F declining in popularity, they decided to kill it.

There was talk in the mid-'90s of an Ace Rimmer spin off from Red Dwarf, between series 6 and 7 when it looked like Craig Charles may not have been able to participate due to the false rape case against him. Ultimately the charges were dropped and Chris Barrie decided he didn't want to commit to any long-term Red Dwarf projects.

I believe someone wanted to do a MWC spin off with Bud and Kelly but they had contracts forbidding spin offs on the characters.
Phil Hartman wanted to do a live action Troy McClure movie, but it didn't happen for obvious reasons.
Matt Greoning also wanted to do a series called Springfield Stories, which would focus on the secondary characters and not the Simpsons. I believe it ultimately wound up as the Simpsons Spinoff Showcase episode. I haven't heard the commentary in a while to have all these details correct, but the gist is there.

Sarah Jane Adventures spun out of a proposed CBBC series that would see the shenanigans of a young Doctor on Gallifrey. RTD rightfully vetoed it on the idea that it guts the character somewhat, and proposed something similarly child-friendly.
Rose Tyler: Earth Defender had buzz for all of two months, but by the time any traction was possible, the buzz was off.

Californication spin-off based on Maggie Grace's Faith character in S6 was set to be developed but it didn't materialise, S6E9 of Californication was set to be the Backdoor Pilot, but yeah it never happened, good job
