I was watching my copy of the Raw Bowl (yes, it is as terrible as Wrestlecrap made it seem) and when the Brooklyn Brawler gives them "the Lombardi Trophy" Vince actually refers to him as "Steve Lombardi? The Brooklyn Brawler?"
Any other times this has happened? (other than those wrestlers who use their real names.)

Thing is, he was known as Steve Lombardi before he became the Brooklyn Brawler. Even after Heenan took him under his wing and made him the Brooklyn Brawler, the other announcers still called him Steve Lombardi sometimes.
When Hulk Hogan went to WCW, he was called "Terry" a few times. I don't remember who said it or the circumstances, but it was one of those "this is so serious, I'm going to use your real name" moments.

Chris Masters real name was used during the trial of Eric Bischoff.

Oh and my contribution - Shamrock calling Undertaker "Mark" when they were going with the whole "Mark Calaway is insane and believes he actually is his character" angle.

We just had this same thread a week or two back. Tons of examples, some notable recent ones being Vince "Can I call you Phil" and CM's "this is Phil Brooks talking to Paul Levesque"

Vince called IRS "Mr. Rotunda" at least once on an early Raw.
Bob Backlund called Diesel "Kevin Nash."
On the Goon's debut, Mr. Perfect outs him as Bill Irwin.
Gladiator Faarooq: Lawler says he "looks like Ron Simmons."
And of course, "THAT'S TOOOOONY ATLAS!"
In the Cactus vs. Douglas match from I think Cyberslam '96, Shane calls himself "The Franchise Troy Martin" on the mic.

I just remember Taker one day cutting a weird promo, all of his promos that lasted more than 5 minutes sucked, in it he was talking about how he was the only guy loyal to Vince, i can't imagine being a good idea telling the viewers that Undertaker was just a character, I bet taker wasn't happy at all.

Wrestlers referring to other wrestlers by their real names is a pet peeve of mine. Why even bother with kayfabe? Like CM Punk referring to "Phil Brooks talking to Paul Levesque". Who is Paul Levesque? Is he Triple H? Why do we call him Hunter then? Is wrestling even real? It just has always seemed like a way of playing with IWC fans.

You could tell 'Taker was REALLY pissed about it. www.youtube.com/watch?v=3mmbvOKZy8gLol, the guy has lived in kayfabe mode for years, I can understand why he would get mad, I read a blog from somebody that used to work for creative, he heavily implied that Taker basically booked himself, that Taker understood his character more than anyone else. I think Mark still thinks he's the character, according to Paul London he used to live in the same town as Undertaker, and the guy had DEADMAN on his license's plates, he aparently used to wear his own merch according to London. Another funny way he keeps kayfabe is the fact that he won't even appear in his wife's instagram, she made a joke about it posing with a figure of Taker. Maybe that's why he was so successful with the gimmick.
