Matt Hardy in a live promo on Raw in the ring saying that Kane and Lita's over one-year love storyline was fake and saying how she came home to him, but then cheated on him with Edge and used Edge and Lita's real names in it.
Not only was it blatantly telling the world that wrestling is fake, but it killed all the momentum his character had by giving such an awful promo (he also said how he hoped they die in a car crash at the end of it to the apathetic response from the crowd)

I'm surprised nobody mentioned that freak accident that killed Owen Hart on live TV. The fact that JR said it was not a storyline or an angle pretty much dekayfabed(is that a word) the rest of the show.

The curtain call comes to mind. Had it not been staged I'd say that time when Matt "broke in" and shat on Lita and Adam in front of a crowd about their fooling around.
Shat meaning shooting on, nothing to do with Kirk or poop by the way.

I didn't watch WCW much during its dying days, but from reading RD's book, it sounds like they openly said things like, "Hey, that's not the finish!" Seems like Russo loved to kill kayfabe during that time. One of the earliest blantant examples was when Fritz Von Erich gave that promo about how Kevin Vaughn was not a real Von Erich and how he never wanted to mention that name ever again.

I think you're mixing your Von Erichs up. Kevin was real(he's the one surviving member of the family, as well). I think you meant Lance, the Von Erich's "cousin".

Iron Sheik and Jim Duggan getting caught with drugs when they were actually feuding on-screen. Maybe not the biggest, but one of the first. That's the first thing I thought of when I saw the thread title. That news was HUGE back then. The two of them were caught in a car with cocaine while they were feuding.

If drugs hadn't been involved, and it was 1997 instead of 1987, they could have easily explained that away with a "Hacksaw is a traitor" angle.

Buff Bagwell and Scotty Riggs discussing their finish on camera? Hehe, that reminds me... Pre Survivor Series '97 I believe it was Shawn Michaels VS.... Ken Shamrock? Hickenbottom was against the ropes and you clearly hear him cue: "Clothesline!" Right before he's Clotheslined over the top.

WCW during the Powers That Be stage pretty much broke kayfabe on a nighty basis. Announcers would throw out kayfabe terms, discuss how things happening is not part of the script, backstage segments would show wrestlers working out a match, etc.

I'd say both TNA's announcers caught reading directly from a script on camera and Sid wanting a "do over" on a live promo both count.

I just used Lance's real name, like Fritz did when he gave that promo. That's why it was breaking kayfabe. Fritz said Vaughn had no relation to the Von Erichs and how they will never be associated with him again and how Fritz never wants to mention the guy anymore. That was big time breaking of kayfabe back then.

HHH being shown on WWE.com as the father of Steph's baby is a big 'un, IMO. Can't Steph and HHH be together in the kayfabe world? I thought it was hinted that they were together kayfabe wise. There was that moment with the illigitimate son reveal, when it looks as though HHH is the son Vince looks at HHH and goes 'but that means you and...' and HHH says he doesn't like it either. And then the 'I would pedigree my own father in law.' and Regal's look after it. They always do those tongue-in-cheek jokes, like whenever Trips and Steph are on camera together, he just blurts out "Hey Steph" for seemingly no reason, and all the "son in law" and "father in law" jokes.
