How about Barry Windham? Horseman and strong US champ in the 80s, World Champ (sort of) in 1993, the Stalker in '96, West Texas Rednecks, out of contract and retirement in '99.
I can't think of a wrestler showed how out of shape they were as much as Windham. He just had a propensity to put on weight and when he was bad he was really bad -- like he looked sloppy (just look at any match he had as the Stalker in that WWF run).
I happen to think Windham is one of the most overrated of all time, personally, but his decline was fairly steep after '94.

I'm suprised nobody mentioned Yokozuna. From becoming World Champion to losing to an up-and-coming "Stone Cold" Steve Austin on a pay per view PRESHOW. He also ended Hulkamania. He lost by "breaking"" the top rope because he was fat. Pretty sad

ECW guys? Seriously, you go from wrestling against Garbage wrestlers at a pool hall to a national television show and that's downward?

Beefcake, Patera and Ricky Morton come to mind. Honorable mention to Lou Albano and Greg Gagne

How about Virgil? Okay, so the most over he's ever been was in his feud with Ted Dibiase(he can make a twig look like a million bucks, pardon the pun), but now he goes to malls charging people $20 to get his autograph!!! Man, those finishers he's taken over the years really damaged his mental capacity.

Terry Taylor ought to be included in this discussion.
Also: Andre. Those 30-second jobs to the Warrior were needless, regardless of his condition. By mid-1991, with the walker and the Bushwhackers licking him, he was just uncomfortable to watch.
And I'll second Taz. His mystique was insane in ECW. He was just a monster. Moving to the "big time" WWF may seem like a step up, but they'd already pretty much neutered him by WrestleMania.

I'd have to say Bret Hart. Once he got to WCW, his career was on a slippery slope.

I'll have to say Tammy Sintch...
From THE hottest, smartest, and coolest girl in all of wrestling...

The AWA (and later WCW) planned big things for "The Cat" Curtis Hughes. (His nickname was based on the fact he was a football player at Kansas State, who are the Wildcats.) But, really and honestly, does anybody remember him aside from his being a lower-card guy (early 90s) and Triple H's butler in the WWF? I say he doesn't fit in with the rest of the group. Don't forget a couple weeks as Y2J's bodyguard in late '99

I'd have to say Bret Hart. Once he got to WCW, his career was on a slippery slope. I dunno about that, he was WCW Champion towards the end of his career, and didn't he get kicked in the head by Goldberg during a WCW Title tournament? That's hardly a decline...

The footage they have of Jake Roberts doing indie shows now on Youtube is truly pathetic and horrible. Rikishi's last run in TNA was godawful as well (albeit short).

Compared to the momentum he had going into his WCW debut that fizzled out quickly, I feel his career made a huge decline.
