Kalisto as Kurt Angle's big acquisition for the cruiserweight division. I have no idea what they were thinking. Kalisto coming out and doing his shitty lucha lucha hand raise thing while the crowd farts all over it was hysterical. Maybe it would have come off better if he still had his "Lucha Satan" theme.

I'd put Cena in the 2008 Rumble in there as a worst surprise. I think a lot of people were looking forward to some time away from him and then the music hit. Wow, STRONG disagree. It's easily one of the greatest surprises in WWE history. Cena was nowhere close to coming back, according to all the reports at the time. And no one for a second, pro writer or fan, suspected that the reports were off in anyway whatsoever. That pop was genuine shock on all levels of wrestling fandom, and a tremendous moment IMHO.

Snuka as the mystery partner at Survivor Series 96. Yes it was the Garden and yes it was just a midcard elimination match. But yeah.
Savio, though it made sense as DX was using the Boricuas as hitmen.
The Nicholas thing I kind of get as it was suggesting Braun didn't need help.
The Samoan Gangsta Party are the newest members of ECW's tag division (led the Arena crowd to chant "Where's our blowjobs?" as this was a really bad show and I guess Heyman knew this so he came out in the middle and promised surprises that, should they let down the crowd, he'd make up for by allowing Divine Brown to give out free fellatio after the show. Yes, THAT Divine Brown...long story!)
Speaking of ECW, Rick Rude is the masked man. Only because we all knew. Then they tried a Russo swerve by having Brian Lee be under the mask at Barely Legal.

It's up there with the recent Brock mitb pop in that it went from surprise/"damn!way to heal quick, wolverine!" To "oh christ. Its another 3 months of cena and another cena rumble win, isnt it?".
