Champ enters the Rumble themselves (wanting to win and "control their own destiny" by picking their own opponent at Wrestlemania or something). Money in the Bank Winner makes a surprise return at the Rumble, cashing in to attempt to win the belt during the Rumble itself using the chaos of the Rumble to their advantage.
So you have one person - who may not even officially be in the Rumble - trying to pin someone while everyone else is trying to compete under battle royal rules. The MITB holder has to keep trying to save the champ from elimination so they can pin the champ, other people in the Rumble would either be trying to help the MITB holder (because they are allies or they think the MITB winner would be easier to beat after winning the Rumble) or stop them from pinning the champ, and the MITB holder would also actively be trying to eliminate people from the Rumble to in turn have a chance to decide who their WM opponent is assuming they win.
This is a dumb idea and yet I talked myself into wanting to see it while writing this.

The story I wanted to see.
Upper midcarder with underdog friend wins MITB.
Underdog babyface gets title shot againt Reigns at Summerslam. Gets killed but wont give up. Gets momentum and actually ends the 2 year reign. Wow.
Usos in, 3 on 1 beatdown.
Their MITB winning friend (who takes the briefcase everywhere) rushes in and takes out the Usos, and Reigns sods off. The two in the ring celebrate. The underdog goes to the turnbuckle to celebrate, the camera faces them on the turnbuckle.
And in the background of the shot, behind their back, their friend hands the briefcase to the referee...

Have a face announce in advance they're cashing in at Wrestlemania and actually follow through with it. I legit thought Daniel Bryan was going to do that to Hall of Pain Mark Henry. Not that the substitute angle led to anything of consequence.

Thi sounds unbelievably convoluted, so I want to see it, but not in WWE's hands because they will not do this well at all.

The champion is in a tag team match, his partner has been knocked out or otherwise incapacitated, and he's currently in trouble. Suddenly, the MITB briefcase holder runs down and wants to cash in at that moment. That adds some drama, does the rival team stop him from winning the belt? Does the tag partner of the champ recover enough to prevent it? And if he wins, you could have some more challengers, like the champion's tag partner in that match, or even one of both of the team that was going against the champion's team.

Have a world champ be the MITB holder (either by winning the MITB or winning the title later without cashing in) and then when they lose the title, they immediately cash in and get it back. I don't think this would be good, but it would be new. Didn't someone do something like that in some indy promotion fairly recently? I recall reading a report describing something like that, where the title shot holder was also the champion at the time.

Considering we've started doing angles of guys just trying to get on the Wrestlemania card, why wouldn't you just hang on to the contract and put yourself at the top of the card?

Heel champ goads face MITB winner into cashing in. Say it's heel champ Lashley and MITB winner Big E. Big E has said that next time he so much as sees Lashley, he's going to cash in. But Lashley is supposedly not in the arena that night, and Big E has a relatively inconsequential match. After the match, Lashley runs in and attacks Big E. After a beatdown, Lashley says, "You promised that the next time you saw me, you'd cash in. Are you a man of your word or a liar?" Big E is basically forced to cash in with a huge disadvantage.

While not Money In The Bank, that's basically what Jax Dane did to win the NWA National Championship at the Crockett Cup a few days ago. Also essentially the finish to Barry Windham and Curt Hennig vs. Chris Benoit and Dean Malenko in the finals of a Double Elimination tournament. Windham and Hennig were undefeated in the tournament, while the Horsemen had a loss. So Benoit and Malenko had to beat the future West Texas Rednecks twice in the match. Benoit and Malenko take the first fall, but Hennig and Windham cheat in the chaos of getting the match restarted and take the deciding fall to win the vacant titles.
