MITB holder forgets to cash in during the 12 months & it becomes void. i'm surprised it took til page 3 for this one to come up. bonus if they throw in an "injury" angle with it. MITB holder gets a 'severe injury" a couple months before the MITB expires and they spent the next couple months desperately trying to get medically cleared before the briefcase expires.

Cash in on vacant world championship. So so who is champ is injured and has to give up belt. Authority figure is about to announce tournament to crown new champ but before he can, MITB holder comes out with lawyer and ref. Under terms of contract, he has to become champion. Isn't that technically how Asuka won the belt from Becky?

MITB holder forgets to cash in during the 12 months & it becomes void. I like this idea. A slight variation on this would be to have the champion properly go after the MITB holder and continually take them out so they could never cash in. It’s always the champion looking over their shoulder, but this would flip it on it’s head. They could then eventually take them out (or think they have) only for them to come back and cash in right before the contract expires.

MITB holder forgets to cash in during the 12 months & it becomes void. Based on an idea I posted earlier, the holder forgets they even have it, and cashes it in with television time remaining, say 10 minutes, and not only does it have to be cashed in, but the holder has to win essentially with time still on the clock, meaning the decision has to happen before that entire 10 minutes expires. MITB holder successfully cashes in, gets the pin with 3 seconds left, is celebrating like crazy, only to be hold they were a mere second off, and thus the title change is reversed. It's hokey, and in essence, a complete screwjob for the current day and age, but it could be a real jumping off point for someone who needs the push (in all honesty, I think the MITB case needs to stop being, for the most part, an automatic title win).

Did Otis ever open it up and reveal his was full of sandwiches, or was that just a joke we came up with?

Someone cashes in and wins, but it’s revealed later that the contract wasn’t in the case so the match is nullified.
Alternatively, someone cashes in and *loses* but reveals that the contact wasn’t in the case so they’ve got another chance.

The winner auctioning it off. It'd be interesting. I know we've established with Ted DiBiase that you cannot sell your title directly to someone, but title shot is another matter, maybe.

R-Truth chasing after the 24/7 champ during the match, seeing the brief case, going up the latter, grabbing it thinking he won, and leaving through the crowd before anyone can grab it away.

There is a treasure map hidden on the other side of the MITB contract, and the MITB winner is consistently forced between easy cash-in scenarios (which would force them to give up the contract and the map with it) or passing up the cash-in attempt to continue trying to find the treasure first.
