Champ enters the Rumble themselves (wanting to win and "control their own destiny" by picking their own opponent at Wrestlemania or something). Money in the Bank Winner makes a surprise return at the Rumble, cashing in to attempt to win the belt during the Rumble itself using the chaos of the Rumble to their advantage. So you have one person - who may not even officially be in the Rumble - trying to pin someone while everyone else is trying to compete under battle royal rules. The MITB holder has to keep trying to save the champ from elimination so they can pin the champ, other people in the Rumble would either be trying to help the MITB holder (because they are allies or they think the MITB winner would be easier to beat after winning the Rumble) or stop them from pinning the champ, and the MITB holder would also actively be trying to eliminate people from the Rumble to in turn have a chance to decide who their WM opponent is assuming they win. This is a dumb idea and yet I talked myself into wanting to see it while writing this. I think would need some very special circumstances to make the MITB holder not like the dumbest person on the planet. Perhaps waiting until the match is close to the end would actually make a ton of sense, especially if the champion draws a late number and thus isn't at a very high risk of getting eliminated early. Perhaps the MITB has an ally that they want to win the Rumble that made it to the end as well, so they would be helping each other in their mutual goals. It could result in a situation where the three people left in the mix are the MITB holder, their ally, and the champion. The MITB hands over the briefcase and makes it official and pins the champion. However, the two heels celebrate a bit too long, allowing the now ex-champion to play possum, and it results in the champion accidentally eliminating their ally, so now one of the Mania main events is the ex-champion and the new champion, with a third person wondering if it was intentional, and perhaps even with a valid dispute of the finish since they weren't eliminated by someone in the match. That would make for a good set up for a triple threat match possibly. As for why they wouldn't secure the Rumble first, it could be a tenuous alliance where the heels don't quite trust each other but have common goals, so the agreement is that the MITB holder cashes in first so that he knows the ally isn't going to cause a DQ once they got what they want.

It’s been 17 years but there is still a lot that hasn’t been done I feel like: -A friend cashing in on a friend (I think this would be perfect on KO and Seth with a slow build) -MITB winner losing by the champ low blowing or intentionally DQing himself -MITB winner vs Royal Rumble winner vs world champ at mania -Cash in after champ vs champ match at survivor series I really like the "MITB winner vs Royal Rumble winner vs world champ at mania" idea! Plus I think they've opened up a few new scenarios with Mania officially staying as a 2 night event. There could be teases with the briefcase on Night 1 but actually cashing in on Night 2.

Heel wins the match by hiding while everyone else does all the work, then slipping in at the end. Afterwards, the get on the mic and declare they don't care about this briefcase; they just wanted to prove how smart they were and to keep these other idiots from winning the thing, and they announce they void the contract and leave the briefcase behind. Just total asshole troll: they win despite doing nothing in the match, and their only motivation was mocking the other participants.
Later in the year, they have a non-title match with the champ. They use blatant low blows or weapon shots and get disqualified, but then they hand the ref a piece of paper, the bell rings, and they pin the champ to win the title. Turns out announcements to wrestling crowds are not legally binding, and you can get other copies of a contract.
In other words, the winner tricks everyone into thinking they don't actually have the contract anymore, so it's a double-surprise when they cash in.
Or, and I'm kinda shocked they haven't done this, the winner sells the contract to someone on the roster, but nobody knows who it is. The champ is a nervous wreck, because they don't know who has the secret weapon to pop up and steal their title.

Have the MITB winner win the royal rumble, Lose to the champion at Wrestlemania. Then immediately attacks the champ right after the match, Cash in the briefcase and win the world title.
Rubber match awaits at the next PPV.

Have a world champ be the MITB holder (either by winning the MITB or winning the title later without cashing in) and then when they lose the title, they immediately cash in and get it back. I don't think this would be good, but it would be new. That's what I'd do if I was booking, well the basic idea I'd change it slightly. Champion is cocky, has an insurance policy, you might fluke a win once but I'll win it straight back. Then they lose the title and the new champion escapes before they can cash-in. They've had the briefcase so long there's only a few weeks to cash-in and the new champion isn't cleared to compete at first, the MitB holder gets increasingly desperate.

Have someone win it then throw it in the garbage the next night on Raw and go "I'll probably just get title shot within the next few months anyway. I'm not carrying around this dumb thing"

Have the championship declared vacant for some reason. MITB winner cashes in, and since there's no current title holder, they automatically become the new champ. It'd be a great move for a chickenshit heel.

1. MITB winner sells the case/contract to the highest bidder (the "auction itself could be a series of feuds between the participants) MITB winner actually gets a title shot some other way, and manages to win the title... only to have the buyer immediately cash it on him.
2. Had this idea when Sheamus had the case. Would have been that Becky Lynch being in a feud with a champ who was eluding her and refusing to give a title shot, and it would have escalated to a point where a backstage argument between her and the title holder leads the champ badmouthing her and Irish people in general and Sheamus overhears it and hands the case over to Becky so she would get her shot at the champ.

It's actually kinda crazy how much they could STILL do with it that would be unique given how long it has been around. With the way WWE always does the same things over and over and over and over and over and over and over and over, I don't find it crazy at all.

Mystery MITB Holder: The winner of the MITB shortly after his victory, comes out without the briefcase and reveals that he's sold the contract to someone for a considerable sum of money and the first contractual title shot after the new holder cashes in. Thanks to a non-disclosure agreement, it is not said who bought it. You could subvert this slightly. Have the storyline go as written, except when the champ loses the belt, it's revealed they actually bought the briefcase, & immediately cash it in & win back the title.

I do wish they'd have MITB winners call their shot, as it were, more often, like RVD's use of the case. Unless it's a face getting revenge on a heel that screwed them over or won the title in a similar fashion, faces should always call their shot.

After all, I remember when Edge held the case for the first time and then won the number one contender tournament, he said that he now has two title shots awaiting him.

Unless it's a face getting revenge on a heel that screwed them over or won the title in a similar fashion, faces should always call their shot. And actually have them win the title match, otherwise it creates a reputation of “faces choke when they call their shot”.

The champ is avoiding the MITB holder as we get towards the one year mark. The last day ends up being on a Sunday with a PPV/PLE, the champ manages to make his escape one last time.
The next night on Raw, the champ is in the ring gloating about how he didn't get cashed in on, when the MITB holder comes to the ring still with the briefcase. He listens to the champ for a few minutes before knocking him out and pinning him to win the title. He then explains that everybody apart from him forgot that it was a leap year and he had an extra day to cash in.

One half of a tag team wins it and the storyline is their partner wants to use it on the tag titles while the winner clearly wants to use it on the world title.

Cash in on vacant world championship. So so who is champ is injured and has to give up belt. Authority figure is about to announce tournament to crown new champ but before he can, MITB holder comes out with lawyer and ref. Under terms of contract, he has to become champion.
