For instance, watching a Raw from 1993, Vince is interviewing IRS and on 3 occassions in a row calls him Mr. Rotundo, before later saying Mr. Schyster twice towards the end of the promo.
Another occassion was Jerry Lawler on Primetime referring to Mr. Perfect as Curt Hennig, but as mentioned in another thread it seemd like Curt Hennig was his name, he was just referred to by a nickname, "Mr Perfect". He's called that multiple times in his career and even debuted as Curt Hennig, so its not much of a reach to say that "Mr. Perfect" Curt Hennig was his full name but Mr. Perfect was said more often to empathize his character.

I recall Ray Traylor calling Curt Hennig “Mr. Perfect” at least once in Nitro.
I have to give AEW a lot of credit that I haven’t heard them slip up and say “Edge” once.

Chris Jericho/Benoit especially by JR. Lucky it didn't happen post 2007. That seemed to happen to everyone. Hell they even had an angle where Lillian announced Chris Jericho the winner of a match that Benoit won and he confronted her the next week about it.

Half the former WWF guys referred to Ray Traylor as "Bossman" on WCW programming, before correcting themselves.

Heenan also called Meng Haku at least once that I remember.

A lot of the WWWF-era guys would call Monsoon Gino instead of Gorilla during interviews/commentary. Blassie, Captain Lou and Ventura seemed to be the worst offenders.
During the ECW Raw in 1997, Vince referred to Stevie Richards as Stevie Ray. Very difficult to tell those two apart.

I recall multiple times Heenan on Nitro would call him Perfect. First two times Tony corrected him jokingly. 3rd time it was just like...who cares. Heenan also called Meng Haku at least once that I remember. It seems like the old WWF guys had a hard time adjusting to the new names. They were so caught up in the world that Vince created that they struggled acclimating to having to shift gears on not just the names of their long time adversaries but their own selves.

Jesse Ventura even called Gorilla “Gino” on commentary at the first WrestleMania. Gino Marella was his stage name at one point in the WWWF and with how the WWF tended to alude to the names as being "stage names" at times, i.e. "Mr. Perfect" being Curt Hennig but rarely referred to as such and "Red Rooster" being Terry Taylor which the whole Heenan feud was based on Taylor being a nobody before Heenan made him the Rooster. Same for One Man Gang and Akeem. They were one in the same as a product of trying to reinvent themselves to better their career. Funny enough, his real name is Robert James Marella, which is not even close to Gino. I write that off as more of an alternate than a mistake or a callback to Gino Marella. I don't think the WWF intended "Gorilla" to be his real first name as he was so far removed from the original Gorilla Monsoon character's original background as being a Manchurian savage turned wrestler. In Kayfabe, you could say, Gorilla was a nickname and Gino was his first name, but in real life and even on a show he hosted unrelated hewas referred to as Bob Marella, which must have been odd for fans familiar with Gorilla to see him on a show but he's not called Gorilla Monsoon.
