I'll start with American Psycho 2. Only connection is that the main character apparently killed Patrick Bateman while he was chopping up her babysitter. Yes, Bateman is killed off in a flashback in which he is so far out of character it's not even funny.
But it had Mila Kunis so it wasn't all bad.

My first thought was Quake 2. It was named as a sequel only for the reason for people to buy but wasn't made to be a sequel.

I think there was a sequel to some movie, based on an old pagan holiday where children dress up and beg for candy. It wasn't the first volume, nor the second, of the series, but I don't think it was the fourth, either.

The Leprechaun movies don't follow on from each other, the leprechaun is pretty much the only constant and in different films he seems to have different powers and possesions (gold, magic flute). Yes i actually sat through all but one of those movies. Maybe Back to tha Hood follows on from In tha Hood but i think i've suffered enough.

Shock Treatment could count. The only things that put it in any sort of timeline with the original Rocky Horror is that Brad and Janet are married rather than engaged, and that two characters who appear in the first 5 minutes of RH appear, now divorced rather than getting married.
The rest of the plot is entirely separate, and doesn't refer to the original at all.

Also, there were plenty of Italian movies that tied themselves into series - I'm sure some sequels to House (the 80s movie, not the 70s Italian one) were like this, and there were definitely Demons sequels like this.

Let me bust open the game sequels bottle since nobody else is.The only two that stick out from my experience are Bomberman 64; The 2nd Attack and Advance Wars; Days of Ruin.
The weird thing with BM64TSA was that they had one character from the first BM64, and he was able to recall a prior battle, but other than that no reference was made. If you're gonna do that, at least put more connections in than saying there was a "prior battle". Hell the character even changes his color and name, you could have completely left it alone.
On the other hand, Advance Wars; Days of Ruin kindof had to both as an evolution of the gameplay, and the fact the current storyline was already inflated and getting beyond dumb.
