It seems like more early wrestlemania's and summerslam's went off without any replacements but I think every Survivor Series and most rumbles had replacements.
Because the replacements weren't scheduled before hand it wound up being a disappointment to fans, I'm sure. With all due respect to these great guys, they just weren't what the fans were expecting.
I would say Scott Casey comes to mind fist, especially since it's Survivor Series time. Tito Santana was a sub (surprise entrant and I don't think Gorilla or Roddy even knew) for Andre the Giant at Rumble 1991. Then there's Savio Vega replacing Shawn Michaels for an In Your House. I think that got lots of boos. He made sense and I like how it was a throwback to Austin and Vega feuding in 1996 but Savio, and I'm a huge Savio fan, was not in a main event class like Shawn. Most people would probably say Bushwhackers and Men on a Mission in 1993 because they wanted Matt Bourne but we already had one match with three guys we didn't know (at the time) so why another one?
What do you guys think?

Doug Gilbert replacing JYD at WrestleWar 89 I forgot about that one. JYD missed two PPVs (WWF and JCP) in less than 7 months.

Don't think any can be worse than Savio.

Booty Man replacing Johnny B. Badd for Uncensored '96 has got to be the worst imaginable last minute replacement. I mean, it's Booty Man.

Nothing against him, but Scott Casey was my first thought. If memory serves, JYD replaced Don Muraco on Jake’s team before he was replaced by Casey...though was Brian Blair mentioned too?

It was Muraco on Warrior's team, replaced by Brunzell and Dog on Duggan's team, replaced by Blair who was replaced by Casey. It's sad the Bees didn't even make the original card. Blair never, ever went back to WWF while Brunzell was a literal jobber on and off until 1993. I always wonder why they didn't just use Lanny Poffo but I guess they didn't want fans seeing the resemblance between he and Randy so close together on the show.

Savio Vega for Shawn Michaels is probably the most egregious especially because they built up anticipation for it.
A sort of random one is Rick Steiner replacing Sabu on a 1995 ECW show.

Meh, it was a nice surprise at the time. (and not technically a PPV) Bringing in the Samoan Gangsta Party was the opposite.

Lanny had turned heel and become The Genius by then I think it was right after Wrestlemania V when he became the Genuis, so still a bit of time. I know Shawn was pretty bad off but there's no way anyone can replace him. WWF should have at least gotten him to put on tights and stand on the apron even if he couldn't even get in the ring. I'd like to be a fly on the wall hearing Vince talk to Savio and really wonder what he, Vince, thought. But then again, the company itself is bigger than any sports entertainer themselves so someone like Shawn can be easily replaced.

Asuka for Nikki Cross at Clash of Champions last year. For a few reasons. First of all, Nikki vs. Bayley was stale as hell, but Asuka vs. Bayley was worse, because they had been wrestling each other all summer. As well, it was a filler match to build to Sasha's return to challenge Bayley and we all knew Bayley wasn't losing the Smackdown Title. Plus, Asuka wrestled Zelina Vega earlier in the night and after the match, Vega beat up Asuka. This was not referenced at all, Asuka came out like nothing had happened earlier and this wasn't her second match of the night. And it had a lame non-finish where Bayley got DQed and Asuka just vanished so they could get the point of the segment.

Lanny had turned heel and become The Genius by then This was Lanny's first appearance as a heel. The Genius vignettes started airing in April. WWF @ New York City, NY - Madison Square Garden - March 18, 1989 (matinee) (20,000; sell out) Televised on the MSG Network - included Tony Schiavone & Lord Alfred Hayes on commentary; featured Lanny Poffo reading a poem about WrestleMania V in which he praised WWF World Champion Randy Savage and said he would beat Hulk Hogan; Poffo then left the ring, opting not to throw his trademark frisbee into the crowd: Prime Time Wrestling - 4/10/89: Paul Roma & Jim Powers defeated the Conquistadors at 13:19 when Roma scored the pin with a missile dropkick WWF Women's Champion Rockin Robin pinned Judy Martin at 9:59 with a roll up after Martin missed a splash in the corner Mr. Perfect pinned Ron Garvin at 12:21 when the momentum of a crossbody off the top by Garvin put Perfect on top for the pin Hulk Hogan defeated the Big Bossman (w/ Slick) in a steel cage match at 11:14 by escaping over the top after handcuffing Bossman to the cage bars, and barely getting to the floor before Bossman escaped out the door; after the bout, Hogan hit Bossman in the head with a chair on the outside, then attacked Slick inside the cage and threw him into the bars (Hulkamania 4, Best of the WWF: Steel Cage Matches, Best of Hulkamania) Shawn Michaels & Marty Jannetty defeated Arn Anderson & Tully Blanchard via disqualification at 13:53 when Anderson pulled the referee out of the ring as Michaels had Blanchard covered following a Rocket Launcher; Anderson reportedly sustained a cracked vertebrae in the match (Allied Powers) The Red Rooster pinned the Brooklyn Brawler at 11:50 with a backslide; stipulations stated Bobby Heenan was barred from ringside for the match Prime Time Wrestling - 4/17/89: Hercules fought Bad News Brown to a double count-out at 14:48 when both men began brawling on the floor and News prevented Hercules from getting back in the ring Jim Duggan & the Bushwhackers defeated Dino Bravo (w/ Frenchy Martin), Jacques, & Raymond Rougeau at the 15-minute mark when Butch pinned Jacques after Duggan hit him with the 2x4 from the floor as Jacques was on the apron

But then again, the company itself is bigger than any sports entertainer themselves so someone like Shawn can be easily replaced. The only problem with that is that Shawn was originally penciled in a man event eight man tag match. So with Shawn being on the apron, his team in Kayfabe would be at a disadvantage. Also, I remember reading the book Titan Screwed that Shawn’s back at that point was very screwed up to the point where traveling may have been very difficult.

I can’t imagine being Jake Roberts and taking the ring with DQ prone Jim Duggan as my co-captain to lead Tito Santana, ancient Ken Patera, and jobber Scott Casey to take on Andre the Giant, Dino Bravo, Rick Rude, Mr Perfect, and Harley Race.
I would’ve turned heel right there.

I remember the Scott Casey one because they didn't change the names on the VHS release of the show.

Also in 1991, Brian Knobbs replaces the Honky Tonk Man in the Royal Rumble. I don’t even think the Nasty Boys had even debuted on TV at that point.
