One that comes to mind in recent memory was on the first episode of AEW Saturday Collision, where Dasha called Buddy Matthews by his WWE name "Buddy Murphy". GCW ring announcer Emil Jay and possibly some other ring announcers pronounce Miedo Extremo as "Miedo Extreemo". The "tre" in Spanish is pronounced as in "Tre' LaMar". I know some people that speak English may not pay attention to this because they don't speak Spanish but considering I speak Spanish, this kinda bothers me a little. He also had difficulty pronouncing Andrade El ídolo's name. I think Kelly doesn't have experience in other languages aside of English. Also basically everyone except Justin Roberts saying Fenix incorrectly In all fairness, Dasha does pronounce "Fénix" correctly as well.

Tony Chimel got better over time, but he used to botch quite a bit, too. There was one of those videos where it captured Cole and Lawler talking during commercial breaks or dark matches or whatever, and they are just tearing into him for screw ups, laughing at him. Dickish, but this would have been years into his tenure, so it's not entirely unwarranted scrutiny.

"Nature Boy" Buddy Rydell (not only was that the wrong name, but Landell was supposed to be a mystery substitute for Dean Douglas) Geez, the one PPV I attended had that, the debut of Xanta Klaus, the hogpen match, the beginning of Razor fears Goldust, King Mabel, Bob Backlund as a Memphis stallfest heel. What a winner I chose! On the flip side you also got the fantastic Bret/Davey match.

I don't think Manny Garcia lasted long after the December 1995 In Your House. You could hear Vince getting upset.
Also, I remember Jim Ross snidely saying something about Lillian announcing someone as an "opponent and challenger."

On the live airing of WrestleMania V, Howard Finkel introduced King Haku as "King Tonga." If you watch on WWE Network or Peacock, you can clearly hear that "King Haku" was added later. There’s actually 3 versions of this. The original PPV King Tonga, then WWF did have Fink fix it for home release and then do it again on the Network or the Anthology DVDs. Think it appeared in a Botchamania video.

There was one night on Raw, Todd Grisham was replacing Lilian Garcia and in his first match, announced a match between Shelton Benjamin and Carlito as a number one contenders match for the Cruiserweight Championship (which wasn't even on Raw, not to mention neither wrestler met the weight requirement lol).

Local Pennsylvania arena announcer Joe McHugh called David Schultz "Mr. D", as opposed to "Dr. D", during his WWF debut match in Dec 1983.
Around the same time, the future Brooklyn Brawler was dubbed "Steve Lombardo" and a young babyface, Backlund protege Eddie Gilbert was referred to as "Billy Gilbert".

At Survivor Series 01, whoever was on massed up and announced both tag champions as WCW tag champs going into the unification match.
When they fixed it for DVD, they screwed up again and got them the wrong way and called the Dudleys the WWF tag champs while they visibly had the WCW ones going in
