Last year, I saw Frankenweenie. It bombed. John Carter was considered a bomb by many. I should note that I had free tickets to these.

Did Ghost Dad or The Rocketeer bomb? Because I saw both of those in theaters.

Dragonball Evolution, which is a shame as I was looking forward to the sequel.

League of Extraordinary Gentlemen, the Avengers (Steed and Peel, not the Superheroes), Mom and Dad save the world (which was actually pretty good), John Carter, Scott Pilgrim (which would have done a lot better if some genius hadn't decided to open it the same week as Expendables) and probably several more I can't remember right now. I had pretty s***e taste in movies when I was a kid, the only good ones I saw in retrospect were usually ones my parents picked out.
not sure if I should include Green Lantern or not, it underperformed but I'm not sure if it qualified as a bomb, and the theater was packed when I went.

I'm not sure if they qualify but I saw Land of the Dead and Jason X in a theater. Jason X it was me and another guy and I saw it twice in theaters. That's the price you pay as a horror geek though.
Saw the Psycho remake with a friend. We were both very disappointed.

Does the Van Damme Street Fighter count?
Also, I assume Pink Cadillac counts($19 million budget, $12 million made). My parents took me with them to see it and we were the only ones in the theater.

Battleship The Hulk (2003) Haunted Mansion Van Helsing Savages Abraham Lincoln: Vampire Hunter (the end of it actually)
I'm sure I've seen more but these are a few that immediately came to my mind right now.

Vegas Vacation (though I liked it and still do) Arthur and the Invisibles (though it did make up its budget worldwide, it was a massive bomb in America)

A lot of people misinterpret "bomb" with "under-perform". Most of these movies made their money back, but didn't make enough money to be worth the trouble. A bomb is when a movie can't even make up for it's budget. Now; THAT is a bomb. Absolutely terrible movie. I wouldn't say that at the time pirate movies were big or anything, but it killed off the idea of anyone even attempting a big budget pirate movie until PotC. Thankfully, I didn't see this in theaters but the first time I spent the night at a friend's house, we watched this since it just came out on VHS and I was pissed. Oh and; I actually have steered clear of bombs, although I really want to say with the cast that they have, Hoodlum from 1997 was probably a bomb. I don't know since the budget isn't listed. It didn't gross much money despite having 3 bonafide stars and Vanessa Williams before she started falling apart. It wasn't very good either, albeit Tim Roth.
