I'm sure there are others from this series that could fit the bill too, but... Star Trek: Deep Space Nine. Episode, "The Valiant". While it would be a realistic thing to happen, a crew of cadets who try to do more than ordered to become heroes, end up all dying and the ship destroyed (save for one cadet and the two series regulars in Jake Sisko and Nog). The episode was also about how their hubris was ultimately their downfall, especially the acting captain who let delusions of grandeur get in the way of sense even roping in Nog. When Jake pointed out that the plan was suicidal, he got thrown in the brig.
They sealed their own fates and I don't think they were that sympathetic. Though back to Star Trek DS9 episodes, "The Siege of AR-558" was really messed up. The characters were trying to survive a horrible situation before finally getting relieved by Federation reinforcements. The characters stationed there were all on edge and the acting commanding officer was way over her head. I think they all ended up dying holding off one last ground assault by the Jem'Hadar, and Nog ended up losing his leg. Nog dealing with his PTSD from the battle was the focus of a followup episode.

There was that one episode of Family Guy where Quagmire just straight up murders his sister's abusive boyfriend. It wasn't a setup to a joke or played for laughs or anything. It was played straight.
Granted, Family Guy (unfortunately) hasn't finished airing yet. But I can't imagine a series finale being that dark for the show. LOST also had a number of episodes with far darker endings than anything that went on in the finale.
Well, I don’t know how straight it was played. Quagmire faked his own death by saying he chokes himself every night and Peter wanted to murder Mort afterward. There was a lot of jokes. Just very dark ones.

Not all lol but yeah, there were a lot of really dark endings. Specifically, any episode where a character died(Time Stands Still(Jimmy gets shot and paralyzed, Rick dies), Rock This Town(JT gets stabbed), Bittersweet Symphony(Cam commits suicide in the green house), Honey(Adam gets in a fatal car crash)). Virtually any Riley-centric episode ended on a dark note, with a few exceptions. All Falls Down has one of the darkest non-fatal endings, with Eli nearly getting stabbed after a prank war goes wrong and Simpson, after spending the whole season trying to be a fair and popular principal, finally snaps and declares the Degrassi version of martial law.

Doctor Who has plenty. One that stands out is The Girl Who Waited. It's very timey wimey, but there are 2 Amy's, one older one younger. They are trying to escape a place. Older Amy is fighting robots while Rory gets his Amy back onto the Tardis. The Doctor sees older Amy, they lock eyes and she knows then what he's going to do and he locks her out of the Tardis saying there can't be 2 Amy's in the Tardis. Rory objects and the Doctor then makes Rory make the choice between the Amy's. He and old Amy say goodbye through the Tardis door, Amy even telling him not to let her in because she would not go gracefully.

Thomas The Tank Engine Henry is sad and he is hiding in a tunnel, they decide to build a brick wall to keep him in as punishment, Gordon being a dick tells him "serves you right" Ringo tops it off by saying "I think he deserves his punishment don't you?" What the f***ing f*** Ringo?

Especially how much Old Amy hates The Doctor, not just because he and Rory left her for 30 years, but also because she sees through his machinations, she sees him with wizened eyes, and she's right. The Doctor really wasn't sure how he was going to be able to keep both Amys in the TARDIS without causing a time paradox, but he had to keep the facade going to rescue Amy. I still cannot, to this day, watch The Angels Take Manhattan. It's just so...yeah. So many feels.

There was an episode of "UFO", the 70's Gerry Anderson series that pre-dated "Space: 1999" that had a dark-ish ending...
A young couple shoot an alien invader as it tries to enter their house in the woods. Turns out the couple was trying to kill the woman's husband and the alien showed up at the wrong time. Because SHADO can't let the truth about aliens get out, everyone involved gets amnesia drugs and is sent away with a cover-up story... Episode ends with woman standing at her husband's grave, then walking into the arms of her lover.
The whole series had a knack for endings that were neither black nor white... sometimes the human race came out a little ahead, and sometimes not.

I’ll just copy and paste my post from a few weeks ago.... An episode of One Foot in the Grave called Who Will Buy? Margaret delivers flowers to the wrong address in a rough area and it’s a sweet blind old man who lives alone. He asks her to read out some letters that were sent to him by his Children and grandchildren in Australia. The letters are actually junk leaflets but Margaret doesn’t want to hurt his feelings so she tells a white lie and makes up a letter and tells it to him. Later in the episode you find out the old man’s house was broken into and he was murdered. He had drawn his life savings from the bank to buy brand new locks for his house but upon hearing the “letter” from his grandkids, he spent that money on toys that he was going to send them for Christmas. STOP posting this you bastard!

The Outer Limits. Tempests.
A crew of a spaceship are returning home with a cure to a disease that is wiping out the planet, the main character knowing that his daughter has the disease and he must return home as soon as possible to save her.
They crash into a gas planet, SPACE SPIDERS attack the ship, and all chaos breaks loose as expected. At one point the main character is bit and then things get weird.
He's suddenly living two worlds. In one, they fend off the spiders, get rescued, and arrive home without further incident, delivering the cure, and being heroes. In the other, they're stuck in space, ship damaged from the fighting, and the creatures are winning. A doctor in the first world tells him it's a reaction from the bite and if he chooses the wrong reality his brain will die. Back in the other one, a friend keeps begging him to wake up and fight.
Making his decision, the main character chooses the dreadful reality, willing to sacrifice himself to ensure the safety of others. He loads himself into an escape pod that has no possible chance of keeping him alive, and aims it for earth to guarantee that, no matter what, the cure arrives even if he must die.
As he declares "I saved them. I saved them."
We cut back to the real world. Turns out, they lost. He's cocooned and being slowly eaten, the spider venom distracting him. There is no one left. The cure will not be delivered. He "chose the wrong reality" and did not "wake up and fight."
They all die. Space. Earth. All.
I was young when I saw it. Heck, it may have been the first dark twist that ever crushed me.

Coach "Out of Control" was an episode showing how obsessed Hayden was with winning and getting endorsements.
- Minnesota State's star cornerback is ruled academically ineligible before the final game of the regular season, their backup is injured, and they have to start a freshman walkon, Cody. He's an 18 year old kid who just wants to play football because it's fun. Oh, and he has to cover the best receiver in college football.
- At the end of the 1st half, Minnesota State is losing, the receiver is owning Cody and Hayden's pissed. Hayden demands that Luther and Dauber get Cody, they try to defend him saying that the whole team is playing bad. Hayden doesn't give a shit, and only wants to talk to Cody.
- Hayden absolutely berates Cody for getting owned. He tells Cody to take out the receiver.
- Minnesota State wins, but Cody did take out the receiver, he hit him so hard he hadn't regained consciousness by the end of the game, and Cody's love of the game has been destroyed.
- At the end of the episode the receiver still hadn't woken up. So Hayden's desire for endorsements led to one kid's career possibly being over, and destroyed another kid's love of football.

I was starting to lose interest in the show towards the end of the first season, but "Duet" changed that. Granted, I don't know if it necessarily was a dark episode, but the topic at hand was dark as could be (establishing that the Cardassians had Nazi-style camps). The fact that Maritza felt he needed to get tried and executed as the camp commander in order to start the healing that he felt Cardassia needed because their crimes stained his own society was...quite terrible in its own way.

Oh, and Battlestar Galactica's final episode was about the only upbeat episode in the series, and it featured main characters murdering other main characters, Rosalyn dying of cancer, and the genocide of 3 of the lines of Cylons.
Having said that, the midseason finale in the final season was one hell of a punch in the gut (they finally get to Earth only to discover it was a nuked-out, dead, former Cylon world rather than a planet full of humans ready to help save the Colonists). The follow up episode was also really, really dark.
